Tear trough rejuvenation is a non-surgical aesthetic treatment for patients with dark under-eye circles or under eye hollowness. Some people are naturally born with dark circles and/or hollowness, however, this is also a process of aging. As we age, the facial fat-pads begin to thin and descend, causing our face to lose the plump lifted appearance of youth and making us look tired. Tear trough filler is not a simple procedure. The area is considered high-risk and should only be performed by highly experienced and trained injectors.
We almost exclusively use Belotero Balance in the under eyes. Belotero balance injectable is a hyaluronic acid (HA) filler that helps smooth moderate-to-severe etched-in lines and wrinkles. We typically don’t use other fillers in the tear troughs because they often create a “Tyndall effect”. The Tyndall effect is a blue discoloration under the skin where the filler was injected. This does not happen with every patient, but it is common because of the result of light scattering from a small optical cavity created when the hyaluronic acid filler reflects light through thin skin. Luckily, this does not happen with Belotero! Upon injection, it immediately integrates evenly into the face. Results are seen right away, and typically last 9-12 months.
Downtime varies from person to person. Since the eye area has such thin skin, bruising is more common than other areas. It is usually mild and lasts less than a week but can last longer. Patients who are using medications that can prolong bleeding, such as aspirin, warfarin, fish oils, or certain vitamins and supplements, may experience increased bruising or bleeding at the injection site. As stated, tear troughs are considered to be high risk because of how many blood vessels are in the area. Risk of vascular occlusion (which we are trained to correct immediately) and issues with lymphatic drainage can occur (which we can easily fix with a little bit of hyaluronidase). Of course, these are the worst-case scenarios, but everyone should be aware of them.

A Hyaluronic Acid (HA) dermal filler, Belotero is the only filler we use in the tear troughs. While Belotero is designed to specifically treat etched-in lines and wrinkles, we find that it’s the best filler in the under-eye area because it does not cause the “Tyndall effect”. The “Tyndall effect”, is a blue discoloration under the skin where the filler was injected. It is common with other fillers, however, Belotero completely integrates into the skin tissue, therefore it does not cause this unsightly effect.
